The San Diego American Indian Health Center (SDAIHC), also known as San Diego Community Health Center, is a 501(c)(3) nonprofit organization, is a patient-centered health home that offers comprehensive medical, dental, behavioral health, and community wellness services available to all people of San Diego.

Founded in May 1979, we honor the vision of community leaders who believed that urban American Indians should have access to community-based health care rooted in traditional cultural values. The legacy of our founders continues nearly forty-five years later as we strive to provide high-quality health care services to a diverse community of all ages.

Patient-Centered Medical Home

As a recognized Patient-Centered Medical Home through AAAHC, our clinic is dedicated to delivering the best possible care—when and how patients need it—in a way that is easy to understand. Our team-based approach ensures care is comprehensive, coordinated across all settings, and centered on each person’s unique culture, values, and preferences. We partner with patients and their families to build strong, trusting relationships and empower them to make informed, healthy lifestyle choices. This model promotes continuous quality improvement and better health outcomes for all we serve.
 

We are proud to announce that on August 25, 2022, we received a three-year accreditation from the Accreditation Association for Ambulatory Health Care (AAAHC). This milestone demonstrates our commitment to delivering the highest quality of care to our patients and maintaining exemplary business practices.

Achieving AAAHC Accreditation reflects that SDAIHC has met the rigorous standards of a nationally recognized third party. Our organization embraces the “1095 Strong” philosophy, ensuring ongoing improvement and quality care throughout our accreditation term.

SDAIHC’s Behavioral Health Alcohol & Substance Abuse outpatient program is licensed by the State Department of Health Care Services (Certification number 370120AN, expiration date 07/26/2025).

Additionally, CARF International has accredited the San Diego American Indian Health Center for three years for its Behavioral Health, Substance Abuse, Integrative Behavioral Health & Medical, and Prevention programs. This accreditation decision reflects our substantial adherence to CARF standards and our commitment to offering high-quality, measurable, and accountable services.

CARF is an independent, nonprofit accrediting body whose mission is to promote the quality, value, and optimal outcomes of services through a consultative accreditation process. Founded in 1966, CARF establishes consumer-focused standards to help organizations measure and improve the quality of their programs and services. Our Vision

We are a community health center dedicated to embodying the values central to American Indian cultures. This includes respect for our patients, acknowledgment of the whole person, and a focus on collaboration to ensure health for the individual and, consequently, for the community. We invite individuals of all tribes, ethnic backgrounds, and walks of life to experience the comprehensive care we deliver and to contribute to the services we provide for children, youth, families, adults, and elders.

Affordable Care for All

We believe that financial barriers should not prevent anyone from receiving quality healthcare. We offer a sliding fee scale to ensure that all our patients, regardless of income, can access the services they need. FTCA and Reporting Fraud
